Harm Reduction in California

California authorizes syringe service programs statewide, and it is simply lawful to possess syringes for personal use. The California Harm Reduction Initiative, created by the 2019 state budget, put $15.2 million into supporting SSPs, at the time the largest government investment in harm reduction in the state's history. That grew into the California Overdose Prevention and Harm Reduction Initiative (COPHRI), a partnership between the state health department, the Sierra Health Foundation, and the National Harm Reduction Coalition that now supports 68 organizations across more than 24 counties and funds frontline staff at up to 72 syringe programs through June 2027. Fentanyl test strips, cookers, pipes, and other supplies are distributed to local programs through the state's own Harm Reduction Supply Clearinghouse, alongside naloxone.

Naloxone no longer requires a standing order at all for the over-the-counter nasal spray version; any pharmacy can sell it directly. A standing order or prescription is still used for other formulations, and pharmacists statewide may dispense naloxone without a patient-specific prescription. California's Good Samaritan protections cover people who possess or distribute naloxone under a prescription or standing order, along with broader protections for anyone who acts in good faith to help someone having a medical emergency.

With this much state infrastructure already in place, the fastest way to find a local program is usually a county public health department's own overdose prevention page rather than a general search.

Harm reduction is a set of practical strategies and ideas aimed at reducing negative consequences associated with drug use. It is also a movement for social justice built on a belief in, and respect for, the rights of people who use drugs.
