Harm Reduction in Virginia

Virginia authorizes Comprehensive Harm Reduction (CHR) programs, which can include syringe access, under a law that got significantly easier to use on July 1, 2020: sites no longer need letters of support from local law enforcement, local government, and the local health director before they can apply, and the state health commissioner can authorize a program in any jurisdiction. Naloxone is available under a statewide standing order from the Virginia Department of Health, and Virginia's REVIVE! program, the state's overdose and naloxone education initiative, has distributed more than 85,000 naloxone kits over five years of State Opioid Response funding to health departments, harm reduction sites, and law enforcement. Fentanyl and xylazine test strips are no-cost through VDH-authorized CHR sites. Virginia's Good Samaritan law offers a defense against prosecution for someone who seeks or obtains emergency medical help for themselves or someone else during an overdose.

The Virginia Harm Reduction Coalition (VHRC) started as a grassroots, peer-run nonprofit and has since grown into a program the state health department itself supports directly, a fairly unusual trajectory that reflects how much Virginia's approach has shifted in a few years. The Department of Behavioral Health and Developmental Disabilities has funded harm reduction work through 40 Community Services Boards statewide since 2018.
