Harm Reduction in South Carolina

South Carolina's syringe service programs sit in a genuine legal gap rather than a clear yes or no. Possessing or distributing paraphernalia is a civil offense under state law, but syringes are notably absent from the list of devices the law actually names, so programs like the nonprofit Challenges Inc. distribute clean syringes, naloxone, and fentanyl test strips without express authorization or express prohibition either way. Bills to formally authorize a "Safer Syringe Program" have been introduced in recent legislative sessions (2023-2024 and again in 2025-2026) but have not yet become law as of this writing, and a similar formalization is underway for drug-checking supplies like fentanyl test strips. Naloxone is on firmer ground: South Carolina's Overdose Prevention Act gives Good Samaritan protection to anyone who gives naloxone to someone they believe in good faith is overdosing.

The South Carolina Harm Reduction Coalition (SCHRC) leads grassroots advocacy and coalition building around this still-evolving legal landscape. On the state side, the Department of Public Health runs LEON (Law Enforcement Officer Naloxone) and ROLL (Reduction of Opioid Loss of Life) to train police and fire agencies statewide, and the COPE program connects overdose survivors with a care team shortly after a naloxone administration.

Because syringe access here depends on a legal gray area rather than a settled law, SCHRC is the most reliable source for which programs are actually operating and how they are protecting participants.
